Medical Weight Loss Program Steps
Step 1: Complimentary Discovery Call
Discuss your goals and see if our program is the right fit for you.
Step 2: Health Coaching
6-Session Health Coaching Bundle: $450
(Includes one 60-minute initial session and five 30-minute follow-ups)
Step 3: Medical Weight Loss Medication (Optional)
- Semaglutide: $200/month (dose-dependent)
- Tirzepatide: $300/month (dose-dependent)
Other Medications:
- AOD 9604, Epithalon, GHK-Cu, Glutathione, MOTS-C, PT-141, Tesamorelin: $135–$150/month
- NAD+: $170/month
- Sermorelin: $160/month
Medication cost includes:
- Telemedicine evaluation & prescription
- 30-day supply of medication at your prescribed dose
- All necessary supplies (vials, syringes/pens, alcohol pads)
- Storage and usage instructions
At Rooted and Well, we do not apply any markup to prescription medication costs. Our goal is to ensure you receive the best possible price, improve access, and make our program as inclusive as possible.
3-month supply shipments may be available depending on state regulations and pharmacy partner participation.
Step 4: Ongoing Support & Follow-Up Care
- Regular follow-up sessions are required to ensure safe, effective management of your treatment.
- We review your medical history, conduct telemedicine exams, and update your treatment plan as needed, including ordering and prescribing medications or labs.
- All recommendations are reviewed and rendered by Elizabeth Al Bayati, FNP-BC, under the supervision of Dr. Joseph Moleski.
Follow-up Sessions:
- 30-Minute Follow-Up: $75 (after completing the 6-session package)
- 60-Minute Follow-Up: $125 (after completing the 6-session package)
- Ongoing phone and email support
Each vial is intended to last approximately 30 days (may vary as dosage increases).
