GLP‑1 Support + Health Coaching with NP Liz
Rooted and Well is a nurse practitioner-led practice owned and operated by Elizabeth “Liz” Al Bayati, FNP‑BC. NP Liz offers personalized GLP‑1 support when appropriate, paired with behavior-change strategies for sustainable lifestyle changes.
Program Overview
You’ll work directly with NP Liz throughout your care. Coaching is virtual, and medication is shipped to most states.
Here’s the general program flow:
Step 1: Schedule Your Free Consultation
Connect virtually with NP Liz to discuss your goals, ask questions, and see if Rooted and Well is a good fit.
Choose ‘Consult’ when booking. A credit card is required to reserve your appointment but will not be charged without your consent.

Step 2: Your Visit
We’ll discuss health coaching, peptide and supplement support, and how these options may align with your health goals, values, and preferences.
Liz will help you make an informed decision about a care plan that feels right for you. As the owner and sole provider of Rooted and Well, she provides personalized guidance based on your individual needs.
A video link will be texted to your phone at your appointment time.
Step 3: Confirm Your Treatment Plan
After your consultation, we will confirm which peptide is right for you, along with your overall treatment plan.
Listed peptide pricing may fluctuate slightly depending on pharmacy availability. I will confirm the best available pricing with you before processing payment. With your consent, the card entered when booking your consultation through the Jane online booking link may be charged. Alternatively, an electronic invoice can be sent, which may be helpful if you plan to submit the receipt for HSA reimbursement.
Credit-card and electronic invoice payments include a 3% processing fee. Venmo, cash, or check payments may be used to avoid this fee.
Step 4: Complete Your Qualiphy Virtual Consultation
You’ll receive a consultation link by email to complete your screening and virtual evaluation through Qualiphy, a telehealth platform with licensed providers serving 48 states.
Consultations are available daily from 6:00 a.m. to 7:00 p.m. PST.
If approved, a licensed compounding pharmacy will prepare and ship your prescription directly to you, typically within 3–5 business days. Rooted and Well will provide guidance, care coordination, refill support, and related nursing services.
If a Qualiphy provider determines that treatment isn’t appropriate, you’ll be refunded any amount paid above the $35 examination fee.
Step 5: Begin Your Medication
Please contact me if you have not received your prescription within 5 days of your exam date or if you have any delivery or storage concerns.
Refer to the attached instructions for drawing up and administering your medication. Keep it refrigerated as directed. If the ice packs are not fully frozen when your medication arrives, refrigerate the medication promptly. *Do not freeze it.*
The following math is optional and for clarification only. Your prescribed dose is the amount of medication you should take, the vial concentration is the amount of medication in each mL of liquid, and the syringe volume is the amount of liquid drawn into the syringe. These are different measurements—always follow your prescription instructions.
For example:
- Prescribed dose: 2.5 mg of tirzepatide
- Vial concentration: 5 mg per mL
- Amount to draw: 0.5 mL
- On a U-100 insulin syringe: 50 units
This example applies only when the vial concentration is 5 mg/mL and the syringe is U-100. Message me with any questions, and do not administer the medication if you are unsure.
Step 6: Follow-Up and Next Steps
- You may receive an email from Qualiphy for an optional medication-safety check-in. The cost is $35, and this check-in does not include refills or dose changes. Please contact me first with any questions or concerns. For emergencies, call 911.
- Health-coaching check-ins with me are expected as part of your treatment. The timing depends on your needs, but a visit 2–3 weeks after starting medication is recommended—especially if you have questions, are not seeing optimal results, or are considering a dose change.
- Please message me when you take your second-to-last injection so there is enough time to complete your consultation and process your refill without a gap in medication.
